Please allow 5 days for mailing prescriptions and 2 days for pick up.
Please have the following information to obtain prescription refills:
Patient name (please spell last name)
Your name and relationship to patient
Daytime phone number where you can be reached
Medication and current dosage
Patient's next scheduled appointment with provider's name
Indicate if prescription should be mailed (provide address,
city, state, and zip code)
or if you will be picking up the prescription
If medication can be called in, please provide pharmacy name and
phone number
If you are experiencing medication difficulties and have a question,
please contact the nurse at (317)575-9111; option #7
Prescriptions can be picked up:
Monday through Thursday 7:30am-6:00pm
Friday 8:00am-4:00pm
The office is not open on holidays and closing time is subject to
change.
